You may find a copy of your invoice within the group's secure Unum website by following the steps below:
Select the "Billing" tab from the drop down, select "Premium Statement;" select the appropriate policy number. The group's most recent invoice will always be the invoice shown on this page. If you need an older invoice, select the arrow under "Select Due Date." Your invoice history will appear, and you may select the appropriate invoice.
Simply Unum / VBCP:
You may find a copy of your invoice within the group's secure Unum website by following the steps below:
Select the "Statements" tab to view current, open invoices, and select the "Current Statements" tab from the dropdown. You will be able to review and complete open invoices from this section. To view completed invoices, select the "Statement History" tab from the drop down. You will be able to review the completed statements and their payment status (Paid/Not Paid).
Employees have 31 days from their eligibility date to elect the Voluntary Life and AD&D coverage, up to the group's non-medical maximum (noted in your benefit summary within the group's contract), without completing Evidence of Insurability.
If the employee does not elect coverage within the initial eligibility period, they will need to wait until the group's annual enrollment period to elect coverage. They will be required to complete Evidence of Insurability for any amount of coverage requested during annual enrollment, as they will be considered a "late applicant." For more information on this provision, please review your group's contract under: "When can you apply for Life insurance coverage if you apply more than 31 days after your eligibility date?"
If your employee is currently covered under the group's Voluntary Life plan, the following examples would qualify for adding dependent coverage:

The employee will need to elect this coverage within 31 days of the event if they would like to add dependents. If they do not elect the coverage within 31 days of that event, they will need to wait for the group's annual enrollment period to elect the coverage, and Evidence of Insurability will be required for any amount of coverage requested.
Please note that if the employee is not currently covered on the plan, the qualifying events mentioned above do not apply. If they are not currently covered, they will need to wait for the group's annual enrollment period to elect the coverage, and Evidence of Insurability would be required for any amount of coverage requested.
